Illinois law [105 ILCS 5/27-8.1] requires:

  • Parents or guardians to provide evidence of completing the required physical examination and vaccinations by Oct. 15 or an earlier date, if so established by local school board policy; AND
  • Schools to exclude students from school until they present proof of meeting the health exam and immunization requirements.

Child Health Examination Forms

  • Students entering kindergarten (or first grade if not previously enrolled in kindergarten), sixth grade and ninth grade at any Illinois public or private school (including charter schools) must provide proof of physical examination on a form approved by both the Illinois Department of Public Health and the Illinois State Board of Education.
  • Students entering an Illinois school from out of state or out of country for the first time at any grade level must also provide the physical exam form.
  • The exam must be completed no earlier than a year (365 days) prior to the start of attendance at an Illinois school.

Dental

  • All students enrolled in kindergarten, second grade, and sixth grade are required to have a dental examination.

Hearing (provided by the school district)

  • Hearing screening is required annually on all children in special education, children new to the district, and teacher/parent referrals.
  • Once a child begins school, hearing screening is required in Kindergarten, first grade, second grade and third grade.

Immunization Requirements

  • The State of Illinois requires vaccinations to protect children from a variety of diseases before they can enter school.
  •  Students must show proof of immunization against up to 12 vaccine-preventable diseases (the number and schedule of these vaccinations depend on a student’s grade and age).

Vision

  • All children enrolling in kindergarten in a public, private, or parochial school and any student enrolling for the first time in a public, private, or parochial school are required to have an eye examination.
    • Examinations must be performed by a licensed optometrist or  medical doctor who performs eye examinations, as specified in Illinois Department of Public Health administrative rules.
  • Vision screening is required annually on all children in special education, children new to the district, and teacher/parent referrals.
  • Once a child begins school, vision screening is required in Kindergarten, second grade, and eighth grade.
